Fantastic form application. Allows us to create a form approval workflow. We are also able to approve and reject customers and notify customers by email if their registration has been approved or rejected by email. Form doesn't get emailed to the email addresses - even though it has been configured to. Sent a message to their support over a week ago and no response. This is a paid app and I would expect better. I now have to uninstall this app, find a new one, and rebuild all the forms. Do not recommend.

Amazing, this App is perfect and fullfill all my needs. Made with brain i would say. Just there is a problem with the VAT field if its required, it allways says i need to fill it even its filled. Same for the phone field if a check the validation, even the format is correct it says its not correct. If this works, you have a new customer. Many thanks.

Had incredibly support in minutes. Worked perfectly for us, especially with hidden fields. We need a tool like this to capture information when a customer wants to order a custom or made-to-order product. This will allow us to do that with ease, and also ease of the customer since we can use the form to capture most information without the customer needing to input it themselves, which saves time for them and speeds up the process.

This is a really nice and easy to use app. I love that the css styling from my theme can be imported automatically. I was having so many issues with Shopify's native forms app and was about to pull my hair out. Then I discovered this app and was really estatic.The developer also has excellent customer service. I had one small bug and the he replied to me within 5 minutes on a Sunday and then fixed my issue in 5 minutes as well. He was super pleasant to communicate with too. And I'm only currently using the free version. If i need more forms or any of the paid features I will have no hesitations paying for it in the future.

A lovely little app which allows you to add multiple forms to your Shopify site. I chose this app as new sign ups are fed directly into my customer base on Shopify and can easily be added into my customer journey without the need to manually export the data. The forms are very much matched to my Shopify theme so they look good and don't look out of place. One of the benefits of this app being build specifically for Shopify stores. I struggled initially to set everything up as I wanted, so was very grateful for the speedy replies and patience of the app founder and developer, Stefano. Also, initially the App could be used with my theme....but Stefano quickly rectified this. Ask you you shall recieve seems to be his mantra :) It is only my first day of using the app, so I am yet to discover the full benefits, but it all looks very promising :)

I'm in love! Originally I just wanted a form. But then I realized that you can include automations. For example "add a tag to a customer" when they fill a form. With Shopify Flow, you can then do ANYTHING you want based on that tag. With these endless possibilities I'm able to Award Loyalty points to a customer for filling a Form. How crazy! And Stefano helped me set it up. Such friendly and timely responses, I'm so excited about the whole experience. Definitely recommended!!

Be careful with this app. I used it because it was free before. But, all at once without prior notice or notification by email or via the app. He simply added paid plans and limited the features and added branding to the form page. to those who had already acquired it for FREE. I don't mind if he charges for the service. he must charge, he has costs. But it's wrong to change the features of those who already purchased the app when it was free. The danger with this app is that you don't know what will happen the next day, you don't know if it will increase the price or restrict you from its features and require you to pay more to use the service you already had. I'm stopping using it. I prefer to use an app that takes we, users seriously and respectfully

Saved me $10USD pm by installing this app. The Shopify native form builder isn't robust enough to handle the info I need for my store. I love that the emails to my team AND the customer can be customised, and the drag & drop form builder is super simple. Support was very quick to assist when I had a question. Impressive! It would be great to have the functionality to duplicate forms, and then edit those. It's a quick process though, so I'm super grateful.

This a great contact form for my Shopify Store! Shopify default form is horrible for the Dawn theme, seriously. This form did everything I needed for my shop and works great! Highly recommend this form and Stefano is great! Very quick response and fixed my very small issue! 5 Stars for sure!

Extremely good and exactly what you need to create the best forms. Especially great is that the styling is taken directly from the theme. Thus, the form fits perfectly into the store without having to configure anything. I was a bit skeptical but it works perfectly. The drag and drop builder is intuitive to use and the forms can be easily created according to your own ideas. There are a lot of fields available - everything you need. There is even a Product Autosuggest field so that the customer can search for and select a product within the form. Very interesting for product-specific questions, for example. Google reCAPTCHA can also be connected with one click. I've been in contact with support and I'm thrilled. The support answered fast and helped me immediately. No wishes left open!
